    Hi, my site is
    http://itcv.info
    when i am on a single job page(i.e. http://www.itcv.info/job/4/web-developer-3-at-foo-inc/#apply ) and i want to apply for a job i fill the form but after i got some errors like

    Notice: Undefined variable: apply_name in /home/content/b/e/r/berniboo/html/itcv/page_apply.php on line 9
    and

    Warning: Cannot modify header information - headers already sent by

    ..any hint pls?
    the server is godaddy
    and i had to eliminate the file .htaccess otherwise is not working (internal server error)
